Inspector Notes

The licensing inspector conducted an investigation in response to a self-reported incident that was received by the department on April 25, 2024, regarding allegations of supervision. The inspection was initiated and completed on April 29, 2024. The inspector was on site from 9:35am to 11:15am. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. One child?s record and two staff records were reviewed.

The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the investigation is determined to be valid. See the violation notice for more details.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Standard 8VAC20-780-340-A
Based on staff interviews, staff failed to ensure the care and protection of a child.
Evidence:
1. On 4/23/2024, around 4:28 pm child #1 (3 years old) was found out front of the child care
center by a parent.
2. Child #1 was found standing on the sidewalk in front of the childcare next to the office
window. About 25 feet from the sidewalk where the child was standing is a neighborhood road.
Plan of Correction: Staff was discharged immediately for not following the schools policy, all parents go directly to the classroom to pick up and sign out their child on the schools attendance classroom sheets.
All staff will retake the child abuse and neglect required training.
The parent sign in-out sheets will be checked daily to insure the schools policy is being followed by all classrooms.
Staff will also have additional training on supervision, guidance and the importance of reporting immediately.
Standard 8VAC20-780-340-F
Based on staff interviews, a child under 10 years of age was not always within actual sight
and sound supervision of staff.
Evidence:
1. On 4/23/2024, around 4:28 pm child #1 (3 years old) was found out front of the child care
center by a parent. The parent had just picked up their child and was loading their child in the
car when the parent observed a child standing on the sidewalk next to the office window.
2. Staff #2 was in the crawler classroom that faces the driveway for the childcare. She
noticed a child standing alone on the sidewalk. Staff #2 said she ran out of the classroom to
the front door the same time a parent was bringing the child back into the childcare
center.
3. Staff #4 said she was in the bathroom in the front room changing child #1's T-shirt, then told the child to go back to the classroom with staff # 3. Staff #4 said she watched the child go into the classroom. Around 4:30pm staff #2 came into the room with child #1, asking why was the child outside alone. Minutes later child #1's parent came to pick up the child. Staff #4 said
she explained to the parent the incident.
