During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was completed on 4/30/2025, as a part of the licensure period. The on-site inspection began at 9:35 am and ended at 10:45 am. The inspector reviewed compliance in the areas listed above. There were (19) children present and (6) staff. The inspector reviewed (5) children?s records and (6) staff records on-site.

This inspection included: document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.
Staff #1, #2 and #3 updated screening was due 1 month ago. The updated screenings have not yet been completed.
Plan of Correction: Staff will make an appointment to get their TB screening.
Standard 8VAC20-780-500-A
Staff hands shall be washed with soap and running water before and after diaper change.
Staff # 5 changed a child?s diaper in the crawler room and did not wash their hands with soap and water.
Plan of Correction: Staff was immediately counseled on proper handwashing.
Standard 8VAC20-780-500-B
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.
Staff # 6 did not clean and sanitize the changing table after diapering child #2.

Standard 8VAC20-780-510-E
Long term prescription drug use may be allowed with written authorization from the child's parent and physician.
Child #5 has written parent authorization for a long term medication since 2/4/2025 but there is no written authorization from the physician.
Plan of Correction: The parent will obtain physician authorization for the medication.

The center may administer prescription medication that would normally be administered by a parent or guardian to a child provided the medication is in the original container with the prescription label in accordance with the prescribers instructions pertaining to dosage, frequency and manner of administration.
Child # 5 has a prescription medication on file with no original container and no prescription label in accordance with the prescribers instructions pertaining to dosage, frequency and manner of administration
Plan of Correction: The parent will provide the prescription label for the medication.
