The center is required to have employees undergo a criminal record check every five years.

Evidence: The most recent determination letter based on fingerprinting for Staff #2 was two months overdue and Staff #4 was nine days overdue.
Plan of Correction: The director will maintain a tracking system to monitor expiration dates and ensure timely renewals in the future.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every
two years from the last screening.

Evidence: The tuberculosis screening for Staff #1 was five weeks overdue, and Staff #3 was three months overdue before subsequent tuberculosis screening was obtained.
Plan of Correction: The director will track TB screening expiration dates trough excel sheets to ensure timely renewals moving forward.
Standard 8VAC20-780-430-A
Furnishings, equipment, and materials shall be of an appropriate size for the child using it.

Evidence: In the infant room, a 13-month-old child was observed in an infant swing. The manufacturer warning on the infant swing stated the swing is not to be used for children that can sit up on their own or are 16 pounds. The child was able to sit up on their own.
Plan of Correction: The child was immediately removed from the infant swing and transitioned to age-appropriate equipment that meet the manufacturer's recommendation.
Infant room staff were reminded and retrained on ensuring that all equipment are used according to age and weight. The director will conduct ongoing monitoring of classrooms equipment use to ensure continued compliance.
