Inspector Notes

Licensing Inspector conducted an unannounced CODE Monitoring Inspection on February 2, 2023. There were 44 children ages7 months to 4 years old present under the direct care of 9 staff. The Licensing Inspector reviewed 5 children?s records and 5 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the Vendor failed to ensure each staff person completes a fingerprint-based criminal history background check prior to 1st day of employment

EVIDENCE:
Licensing Inspector reviewed 5 staff records. Record for Staff #4 did not include a fingerprint-based criminal history determination letter. Date of hire for Staff #4 is 3/14/2022.
Plan of Correction: Staff will schedule and complete a fingerprint background check. Vendor will submit proof of correction to the Licensing Inspector within 10 days.
