Inspector Notes

An unannounced inspection was initiated on 11/20/2024 and completed on 01/14/2025 in response to a complaint received by the licensing office on 11/18/2024 relating to allegations of supervision and parental involvement and notifications. The inspector reviewed 1 child?s record and 1 staff record onsite on 01/14/2025. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

The vendor shall maintain a written record of children?s serious and minor injuries in which entries are made the day of occurrence.

Child 1 sustained injuries on 11/13/2024 and 11/18/2024; there was not a written record of the injuries available for review.
Plan of Correction: Will make sure staff complete injury reports for all injuries.
