Each person in a supervisory position has been certified by a practicing physician or physician assistant to be free from any disability which would prevent them from caring for children under their supervision.

There was not a record of a health exam on file for Staff 2. The health exam for Staff 3 was 2 years overdue.
Plan of Correction: Will set appointments.

The vendor shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities, and dietary restrictions. The list shall be dated and kept confidential in each room or area where children are present.

Staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB); documentation of the screening shall be submitted at the time of employment, prior to coming into contact with children, and shall be been completed within the last 30 days.

There was not a TB screening available for Staff 1 nor Staff 2. The TB screening for Staff 5 was completed 9 months before being employed.
