During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 08/05/2025, as part of the licensure period.
The on-site inspection began at 12:45 pm and ended at 1:55 pm. The inspector reviewed compliance in the areas listed above.
There were 4 children present and 2 caregivers; the point total was 8. The inspector reviewed 3 children?s records and 2 caregiver records on-site.
This inspection included document review and tour of the facility.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 08/18/2025. A POC submitted after this date will not appear on the public website.
Violations
6The Tb screening in the record for Caregiver #2 was overdue for three months.
Potentially poisonous substances, materials and supplies such as cleaning agents, shall be stored in areas inaccessible to children.
Cleaning products were observed in an unlocked cabinet under the care area's kitchen sink.
The record for Child #1 was missing the parent's work phone number and work address.
The annual review in the record for Child #3 was overdue by almost three months.
The provider shall review the emergency plan at least annually, update the plan as needed, and document in writing each review and update to the emergency plan.
The emergency plan review was overdue by seven years and nine months.
The menu was not dated as required.
