Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A renewal inspection was initiated on August 26, 2021 and concluded on August 26, 2021. The provider was contacted by telephone and a virtual inspection was conducted. There were two children present, ranging in ages from 15 months old to four years old, with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records, one caregiver, and one household were reviewed

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the provider did not ensure that one caregiver had current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

Evidence:
1. The record for caregiver #1 had documentation of CPR certification with an expiration date of 9/10/20.
2. The provider confirmed the certification had expired.

Standard 22VAC40-111-180-A
Based on record review, the provider did not obtain a current report of tuberculosis screening for each household member every two years from the date of the first screening.

Evidence:
1. Household Member #1 had a report of tuberculosis screening (TB) form which expired on 10/03/20.
2. The provider confirmed that the TB form had expired.
