Inspector Notes
An unannounced, on-site focused inspection was initiated on November 27, 2024, and completed on November 27, 2024, as a part of the provisional licensure period. The on-site inspection began at 11:50 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were eight children present and one caregiver; the point total was 12. The inspector reviewed six children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
9The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no documentation of a physical exam for Child #1and child #4, both who had been enrolled for over a year.
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. The record of household member #1, identified as a substitute provider, contained documentation of 4 hours of training during 2023 when 16 hours was required.

Wood burning stoves and fireplaces and associated chimneys shall be inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed. There was no documentation of an inspection for the wood burning stove in the past 12 months.
The provider shall maintain a child's record that contains all required information. The record of child #1, child #2, child #4, and child #6, did not contain the policy number of the child's medical insurance. The record of child #2 and child #4 did not contain the telephone number of each custodial parent's place of employment. The record of child #5 and child #6, did not contain the written authorization for emergency medical care should an emergency occur, and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds. The record of child #5 did not contain the name, address and telephone number for a second designated person to contact in case of an emergency if the parent cannot be reached.
A working portable battery-operated weather band radio, and extra batteries shall be kept in a designated area and be available to caregivers at all times. The portable battery-operated weather band radio on site was not operational.
