Inspector Notes
An unannounced, on-site focused inspection was initiated on November 27, 2024, and completed on November 27, 2024, as a part of the provisional licensure period. The on-site inspection began at 11:50 am and ended at 12:40 pm. The inspector reviewed compliance in the areas listed above. There were eight children present and one caregiver; the point total was 12. The inspector reviewed six children?s records and two caregiver/household member(s) records on-site.
This inspection included document review, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Repeat/ Systemic Deficiency
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance. There was no documentation of a physical exam for Child #1and child #4, both who had been enrolled for over a year.

Each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department. Documentation of immunizations for child #3, was not signed by the physician.

In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities. The record of household member #1, identified as a substitute provider, contained documentation of 4 hours of training during 2023 when 16 hours was required.

The record of child #1, child #2, child #4, and child #6, did not contain the policy number of the child's medical insurance. The record of child #2 and child #4 did not contain the telephone number of each custodial parent's place of employment. The record of child #5 and child #6, did not contain the written authorization for emergency medical care should an emergency occur, and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds. The record of child #5 did not contain the name, address and telephone number for a second designated person to contact in case of an emergency if the parent cannot be reached.
