Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. This inspection report was amended on March 3, 2026.
An unannounced, on-site monitoring inspection was initiated on September 29, 2025, and completed on September 29, 2025, as a part of the licensure period. The on-site inspection began at 3:19 pm and ended at 4:40 pm. The inspector reviewed compliance in the areas listed above. There were two children present and one caregiver; the point total was four. The inspector reviewed two children?s records and two caregiver/household member(s) records on-site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Standard 22.1-289.057-A
The family day home is required to submit to VDH and implement a plan to test potable water for lead. The family day home did not have evidence of submitting a testing plan to VDH. The provider stated she had not submitted a plan.
Plan of Correction: Per the provider: I will complete the water testing.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Report of Tuberculosis Screening form every two years from the date of the first screening. 1. The record of caregiver #1 contained documentation of a TB screening that expired one month ago. An updated screening had not been obtained. 2. The record of caregiver #2 contained documentation of a TB screening that expired one month ago. An updated screening had not been obtained.
Plan of Correction: Per the provider: We have scheduled to get updated TB screenings.
Standard 8VAC20-800-800-B
The provider shall review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.1. The most recent review of the emergency plan was dated 08/27/2024. 2. The provider stated that the annual review had not been done.

Standard 8VAC20-800-830-A
The emergency evacuation procedures shall be practiced monthly with all caregivers and children in care during all shifts that children are in care. There was no documentation of evacuation drills for the period of February through August of 2025.
Plan of Correction: Per the provider: I will record the drills every month.
Standard 8VAC20-800-920
When meals or snacks are provided by the family day home, the menu for the current one-week period shall be dated and posted or placed in an area accessible to parents. The provider stated she had not completed a menu for the current week of September 29, 2025.
