REPEAT VIOLATION
Each staff member shall submit documentation of a negative tuberculosis (TB) screening every two years from the date of the initial screening or testing.
On the date of the inspection, Staff #1 was overdue for a subsequent TB screening/test by 2 months. The provider stated Staff #1 had a screening/test performed and was waiting for the results.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following: work phone number and place of employment of each parent.
