Inspector Notes

A monitoring subsidy inspection was completed from 9:55 until 12:35. There were 26 children present with 5 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 7 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Standard 8VAC20-790-520-F
The vendor failed to maintain in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions. This list shall be dated and kept confidential in each room or area where children are present.
Evidence:
There were children in the toddler and pre-k room that had food sensitivities and there was not a list in each room or area where children are present.
Plan of Correction: The director will use the model form to update information for each area.
Standard 8VAC20-790-770-B-4
The vendor failed to given only medication at the dose, duration, and method of administration specified on the manufacturer's label for the age or weight of the child needing the medication.
Evidence:
1. Child #6 (DOB 7/18/21) was administered Tylenol on 1/13/23 and the bottle stated it was only for children under two; a physician had not authorized this usage. 2. Child #7 (DOB 8/26/21) was administered Tylenol on 2/2/23 and a physician had not authorized the usage.
Plan of Correction: The director decided the center would not administer medications.
