Inspector Notes

An unannounced monitoring inspection was initiated on 7/14/2025 and completed on 7/28/2025 in response to a self-report received by the licensing office on 7/11/2025 relating to the staffing and supervision at the center.. This inspection included document review, tour of the facility, interviews, observations and measurements
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to Supervision as well as Special care and provisions and emergencies, and are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 7/9/2025 at approximately 4:30pm, a child age 6, was in the main SACC room with a group of 12 children and 3 staff. The child exited the room, located on the second floor, walked down the hall, down the stairs and out the front door without SACC staff noticing. The staff at the front desk at the community center noticed and went outside to stop the child. When the child failed to stop the staff returned to the front desk and alerted the SACC staff.
Child #1 went approximately 88 yards down the side of the building, crossing into the parking lot where a farmers market and fair was in progress.
Plan of Correction: Child was assigned a staff to be with them throughout their time in the program. The staff will be on a one-to-one basis with the child and the staff will rotate with other staff members on an hourly basis.

Children under 10 years of age always shall be within actual sight and sound supervision of staff.

On 7/9/2025 at approximately 4:30 pm a child, age 6, was out of sight and sound supervision when the child left the SACC room and walked downstairs out of the building and 88 yards down the side of the building, crossing into the parking lot where a farmers market and fair was in progress. The child was out of sight and sound supervision for approximately 3 minutes.
Plan of Correction: Staff will escort children to and from the restroom and there will be supervisory staff placed in the hall near the stairs to further ensure no child can enter or exit the hallway unsupervised

The center or other appropriate official shall notify the parent immediately if a child is lost,

On 7/9/25 a child #1 was out of sight and sound supervision of SACC staff for approximately three minutes. The center or other appropriate official failed to notify the parents immediately of the incident. Parents were notified the following day
Plan of Correction: All parents will be notified of any and all incidents involving their children while here in the program the same day of the incident . Parents will be notified the same day.
