Inspector Notes
This monitoring inspection was initiated by licensing staff and concluded on 10/05/2023, between the hours of 11 am/pm to 12:30 pm. There were 5 children present (17 points), ranging from infant to preschool-age, with 3 caregivers supervising. The children were engaged in the following: games, bathroom breaks, hand washing, diapering, song, music, outdoor play, lunch, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 5 children records, 3 staff records, and 2 household records were reviewed.
Information gathered during the inspection determined that there were (3) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

1. On the date of inspection (10/05/2023), the last documented review of the emergency contact information for Child #1 was dated - 05/05/2022.
2. The last documented review of the emergency contact information for Child #4 was dated - 04/28/2021.
Evidence -
1. On the date of inspection (10/05/2023), the following children did not have documentation that each had been adequately immunized within their files: Child #2 (start date - 09/05/2023), and Child #3 (start date - 09/25/2023).
2. While an immunization page was missing entirely for Child #2, on the health forms for Child #3 their immunization page had a note from their doctor's office saying, "see attached." However, no other immunization documentation was attached to this form.
Evidence -
1. On the date of inspection (10/05/2023) the following children did not have an updated immunization page available for review: Child #1 (last on file - 05/05/2022), and Child #5 (last on file - 03/07/2023).
2. On the date of inspection, both Child #1, and Child #5 were under the age of two years.
