Inspector Notes

This is an addendum to a Family Day Home renewal inspection report dated - 03/22/2024 for this Provider. After the inspection had concluded, a review of the notes taken indicated that two separate violations were not noted on the issued violation notice. Therefore, this addendum will serve has documentation for the violations not accounted for on the original notice. Please see violation notice for more information.

Standard 22.1-289.036-B-2
Based on review, 1 of 2 household files indicated a Fingerprint check (FPC) on file at the Family Day Home (FDH) was older than 5 years in age.

Evidence -
1. It was later discovered, after a review of notes taken on site, that a FPC for Household Member #1 was dated - 10/09/2018.
2. This finding expired on 10/09/2023 and will need to be updated.
Plan of Correction: A fingerprint search will be scheduled in order to obtain an updated finding for Household Member #1.

Based on review, 1 of 2 household files indicated a CPS check on file at the Family Day Home (FDH) was older than 5 years in age.

Evidence -
1. It was later discovered, after a review of notes taken on site, that a search of the central registry (CPS check) for Household Member #1 was dated - 10/29/2018.
2. This finding expired on 10/29/2023 and will need to be updated.
Plan of Correction: A request of the central registry will be submitted in order to obtain an updated finding for Household Member #1.
