During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

This inspection report is an addendum to the unannounced renewal inspection conducted on 03/20/2025. The Provider was given an opportunity to supply documentation of training for all staff prior to the end of the annual training period.

Information gathered during the inspection and up until 04/10/2025, determined non-compliance with an applicable standard, and this violation was documented on the violation notice issued to the program.

Caregivers shall obtain a minimum of 16 clock hours of training annually.

None of the caregivers (#1,#2 and #3) obtained any training hours in 2024.
Plan of Correction: All applicable staff will obtain 16 hours of annual training.
