Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts and documents review.
A renewal inspection was initiated on 9/30/2021 and concluded on 9/30/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 15 children present, with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not obtain a fingerprint based background check for each staff member prior to their first day of employment.

Evidence:
The records of Staff #1 (DOH: 08/16/2021) does not contain fingerprint based background check results required prior to their first day of employment.
Plan of Correction: Fingerprint based background check results for Staff #1 have been received and are on file.

There was an extended delay due to a processing error.

All other PK staff background check documents were on file at the time of inspection and can be provided for review.

Additional checks have been added to our internal process to ensure we maintain compliance.
