During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An inspection was initiated on 1/29/25 and completed on 2/26/25 with local agencies in response to a self-report received by the licensing office on 1/14/25 relating to forbidden actions. The inspector reviewed one child?s record and one staff record electronically on 1/29/25. This inspection included document review, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self report, violations that were not reported were found related to administration and behavior guidance and are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 3/10/25. A POC submitted after this date will not appear on the public website
Violations
3The center?s child guidance and classroom management policy (discipline policy) requires that staff clearly state expectations for appropriate behavior and provide alternative choices and redirection away from inappropriate behavior. On 1/10/25, in response to children?s inappropriate behaviors in the classroom, Staff #1 responded to children with an elevated voice and physical intervention without stating expectations and alternatives for more appropriate behavior choices.
On 1/10/25, Staff #1 used an elevated voice and physical intervention to address preschool children who were not behaving in an appropriate manner in the classroom.
On 1/10/25, a child was sitting at a table when Staff #1 grabbed the child by the wrist and arm to get them out of the chair. Child #1 resisted, and Staff #1 picked the child up and carried the screaming child through another classroom, into a bathroom.
