During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on 10/10/25. The on-site inspection began at 9:40am and ended at 11:55am. The inspector reviewed compliance in the areas listed above. There were 21 children present and five staff. The inspector reviewed five children?s records on-site and five staff records electronically on 10/17/25. This inspection included document review, a tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/27/25. A POC submitted after this date will not appear on the public website.

Repeat Violation
The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.
There was no documentation in the record of Staff #2 to show that the Virginia Department of Education-sponsored orientation course was completed.
Plan of Correction: Assigned staff will complete the proper documents and place in their folders.

Repeat Violation
Areas and equipment of the center, inside, shall be maintained in a safe condition.
In the kitchen, there were cabinets with a label that stated "mold." The lunch to be served on the day of the inspection was observed sitting directly on top of the cabinets.
Plan of Correction: Assigned staff will make sure that the buildings are in a safe condition.

At least one staff in each classroom or area where children are present shall have current certification in cardiopulmonary resuscitation (CPR) as appropriate to the age of the children in care.
Staff #2 and Staff #3 were observed working together with a group of children in a classroom. Neither staff had current CPR certification.
Plan of Correction: All staff will have the proper trainings that are required to work with children.

There was no documentation to show that the center completed a monthly evacuation drill in August through September 2025.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain all the required information.
The records for Child #1 and Child #2 did not contain all the required information.
Plan of Correction: Tlre assigned staff will update children's records and add all proper information that is needed.

Staff records shall be kept for each staff person and contain all the required information.
The records for Staff #1 and Staff #2 did not contain documentation to demonstrate that the individuals possess orientation training.
