Inspector Notes
An unannounced monitoring inspection was conducted today, November 9, 2022. There were 3 children (11 points) present with 2 caregiver's supervising. The children were observed napping and eating snack. The house, cribs, cots, menu's, attendance logs, evacuation logs training logs, first aid kit and carbon monoxide detectors, children's records, caregiver records and household member records were reviewed. In the daycare room, the outlet on the front wall was missing a protective cover.
Evidence:
Child #1's record does not contain documentation of names of persons other than the custodial parents who are authorized to pick up the child.
Child #2 and #3's records do not contain documentation of the telephone number of each custodial parents place of employment.
Evidence:
Child #3's record did not contain documentation of immunization on the first day of attendance. First day of attendance was10/24/2022.
