Inspector Notes

An unannounced renewal inspection was conducted on May3, 2023. There were 4 children present (13 points) with 2 caregivers. The home, children's records, caregivers records, household member records, attendance logs, evacuation logs, cribs and emergency preparedness procedures were reviewed. The children were observed finishing lunch and playing with floor toys. There were no medications on site. Areas of non-compliance were cited on the violation notice. Based on a review of household member's records, the provider failed to obtain fore each adult household member a current Report of Tuberculosis Screening form, every two years from the date of the first screening.

Evidence:
Household Member #1's record does not have documentation of a current Report of Tuberculosis screening form. Last screening 02/21/2019.
Plan of Correction: I will have household member submit TB screening.

Based on a review of children's records, the provider failed to obtain all require documents for each child's record.

Evidence:
-Child #3 and #4's records do not contain documentation of the phone number of each custodial parent's place of employment.
-Child #4's record does not contain documentation of the addresses of two designated persons to contact in case of an emergency, if the parent's can not be contacted.
