The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical exam record available for Child #2, who has been in care for over 35 days.
Plan of Correction: The parent will be contacted in order to obtain documentation of a physical examination for Child #2.
Standard 8VAC20-800-60-B
A child's record shall contain the following information: Written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.

Written authorization for emergency medical care was not signed by the Parent of Child #2. Child #2 has been in care for over 1 month.
