Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program. A monitoring inspection was initiated on 01/06/2022 and concluded on 01/14/2022. The director/provider or in-charge person was contacted by telephone to initiate the inspection. There were 17 children present and 5 staff. The inspector emailed the director/provider a list of items required to complete the inspection. The Inspector reviewed 1 child's records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

1Evidence:
Staff #1, hired 12/01/202,1 indicated on the sworn statement, having lived in 2 states outside of Virginia in the past 5 years. Staff #1 did not have documentation of having a criminal record check or sex offender check for either of the 2 states listed.
It was difficult to find how to obtain these checks. The admin staff will be working on obtaining this information this week.
