Standard 8VAC20-780-350-C
Based on interviews and a record review the center did not ensure when children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.

Evidence:
On 02/09/2024 for approximately 20 minutes between 7:55am and 8:15am, Staff #1 had a group of 20 children ranging in age of 3 years-5 years with no other staff present.
a. The ratio for the youngest child who was 3 years-old is 1:10.
b. Attendance sheets confirmed staff #1 had 20 children my 7:55a.
c. Staff and the Director confirmed that the center was out of ratio from 7:55am-8:15am.
Plan of Correction: The director was not in the building at the time of the incident. The next day she was informed of the incident. The director told staff that she should've been notified because there was enough staff int he building where she did not need to be out of ratio.
