Inspector Notes

An unannounced inspection completed on 01/09/2024 in response to a complaint received by the licensing office on 12/18/2024 relating to allegations of Special Care Provisions and Emergencies. The inspector reviewed records of any involved staff and/or children. parties. This inspection included: document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-550-O
The center shall notify the parent by the end of the day of any known minor injuries.

On October 11th The center did not notify the parents of a 2 year-old that their child had tripped and hit her head on the wooden ground while on the outside play area. After picking child #1 up from the center, the parents contacted the center following the discovery of a bruise on the right side of their child's forehead. Staff confirmed the child had fell and hit her head and that she had failed to provide them with an injury report.
Plan of Correction: Staff was terminated. The owner called a meeting to discuss and do training on injury reporting, how to fill out injury reports, the importance of notifying management and parents of injuries promptly and the seriousness of head trauma.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence and include all required information.

Injury report dated 10/17/2024 for child #1 who was bitten by another child did not include the following required information:
a) Staff and parent signatures or two staff signatures
b) Date, time and how the parents were notified
