Inspector Notes

A monitoring inspection was initiated and concluded on 8/24/2023. There were 32 children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. The inspection started at 1:40 pm and concluded at 5:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on a review of records and interview, the licensee did not obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years.
Evidence:
The record for Staff #1 (date of Employment 8/1/2023) contained a Sworn Disclosure Statement which documented that Employee #1 resided in Pennsylvania within the preceding 5 years.
Employee #1's record did not contain copies of requests or results of an Abuse and Neglect Registry search for Pennsylvania.
Employee #1's record did not contain copies of requests or results of a sex offender registry check for Pennsylvania.
Employee #1's record did not contain results of a Criminal History Information Check for Pennsylvania.
Plan of Correction: The licensee will request the Abuse and Neglect Registry, sex offender registry check search and Criminal History Information Check from Pennsylvania immediately and will send proof that these have been submitted within 10 days of the inspection.

**REPEAT VIOLATION**
Based on a review of records and interview, the licensee did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Evidence:
Five children's records were reviewed.
The record for child #5 who began attending the program 7/2/2020, did not contain any documentation of immunizations received.
The program director confirmed that this documentation was not present in the record.
Plan of Correction: The program director will contact child #5's parent this week to request that child #5's immunization record be submitted by the end of next week.
