Inspector Notes

An unannounced monitoring inspection was conducted between the hours of 9:00am through 10:05am. There were a total of 6 children present, ranging in age from 9 months to 3 years (15 points) with the Provider. A total of 2 children?s files, 1 staff file, and 1 household member?s file were reviewed. The children were engaged in the following: eating snack, practicing writing, coloring, and engaging in free play. Positive interactions between the caregiver and children were observed during this inspection. A complete inspection of the programming space, children and staff records, emergency drills, attendance, and emergency procedures were completed today. The provider advised there was no medications on site today. There was an adequate supply of age appropriate materials and toys. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not obtain updated Virginia Child Abuse and Neglect Central Registry results for staff and adults living in the family day home as required every five years.

Evidence: Staff #1 and Adult Household Member #1's most recent Virginia Child Abuse and Neglect Central Registry results on file were dated 2/7/2018.
Plan of Correction: Contacted CRS and working to resolve issue with pending checks.
Standard 8VAC20-800-250-C
Based on observation, the provider did not ensure that toys were not hung from the sides of the crib when either the child begins to push up on hands and knees or is five months of age, whichever occurs first.

Evidence: Child #5 (Age: 9 months) was observed sitting in the crib located in the main play area at the beginning of the inspection and there were toys hanging down on connected plastic links spanning from either side of the crib.
Plan of Correction: Removed rings.

Based on observation and provider interview, the provider did not ensure that cribs were not used as a play space for infants.

Evidence: Child #5 (Age: 9 months) was observed playing in the crib in the main play area at the beginning of the inspection. The infant was sitting up in the crib with a boppy pillow. There were various toys located in the crib with the infant including a small bucket, a small mirror with shapes toy, a soft doll, a blanket, and two stuffed toys.
Plan of Correction: Removed all items.
