Inspector Notes

A monitoring inspection was conducted on 4/4/2022. The provider and two assistant caregivers were caring for 11 children, totaling 18 points. The Inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies, and nutrition. A total of 3 children's records and 3 caregiver records were reviewed.

The children were observed finishing morning academic activities and eating snack.

Standard 8VAC20-800-100-A
Based on a review of children's records, it was determined that the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
Child B has a documented date of enrollment of 7/6/2020. There is no physical exam in the child's record.
Plan of Correction: I will remind the parent to return the completed physical form today.
Standard 8VAC20-800-180-A
Based on a review of caregiver records, it was determined the provider did not obtain for each caregiver a current Report of Tuberculosis Screening form, in accordance with the requirements in 8VAC20-800-170, every two years from the date of the first screening.
Evidence:
The TB screening for caregiver C expired on 3/18/2022.
Plan of Correction: I will make sure she gets a repeat TB screening done this week.
Standard 8VAC20-800-60-B
Based on a review of children's records, it was determined that two children's records do not contain all required information.
Evidence:
1 - Child record A does not include the information for two emergency contacts other than the parents.
2 - Child record B does not include documentation showing that the parent has reviewed the child's contact information within the past year. This file was most recently reviewed by the parent on 6/24/2020.
Plan of Correction: Both parents will be asked to update the information today.
