Inspector Notes

An unannounced monitoring inspection was conducted today between 11:30am and 12:45pm. There were 10 children present with the Provider and 1 assistant for a total of 22 points within the supervision guidelines. The children were observed playing freely, reading, eating lunch and transitioning to naptime. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during todays inspection and can be viewed on the Violation Notice.

Standard 22.1-289.011-F
Based on observation, documents required by the Superintendent were not posted in a conspicuous place on the licensed premises.
Evidence:
1. The providers most recent inspection was not posted at the family day home.
2. The providers license was not posted at the family day home.
Plan of Correction: I will print them.
Standard 8VAC20-800-430-A
Repeat Violation
Based on observation, children under two years of age had access to stairs.
Evidence: There is a downward staircase that the children have access to where the program takes place. There is a gate at the top of the stairs, however the gate is only secured on one side and swings open without a hook, latch or lock. There was a one year old observed in care today.
Plan of Correction: I will start securing it closed.
Standard 8VAC20-800-480-D
Based on observation, equipment used by children was not used in accordance with the manufacturers instructions.
Evidence: At the start of the inspection there was a one year old child observed sitting in a bouncy seat on the floor. The child was not buckled into the chair.
Plan of Correction: I don't buckle it so that [the child] can freely get out on their own. I will start buckling if they are in the chair.
Standard 8VAC20-800-90-C
Repeat Violation.
Based on record review, the family day home did not obtain documentation of additional immunizations for each child under the age of two years.
Evidence: Child #10 (22 months) most recent documented immunizations are dated 4/18/2023.
Plan of Correction: I will ask the family for the immunizations.
