Violations

3
Standard 8VAC20-800-50-A
Based on interview, the family day home does not keep a written record of children in attendance each day.
Evidence: The provider could not produce documentation of daily attendance.
Plan of Correction: I will start doing it.
Standard 8VAC20-800-530-A
Based on observation, rest mats did not consist of a top and bottom cover when being used by napping children.
Evidence: Five out of five children observed napping did not have a bottom linen on their rest mats.
Plan of Correction: I didn't know that the soft mats needed a bottom sheet. I have extra so I will start using them.
Standard 8VAC20-800-60-A
Based on interview, the provider did not maintain a record at the family day home for each child.
Evidence: The provider stated that they did not have a record for child #5.
Plan of Correction: Today the parent asked to have them here for an emergency. I will tell parents they can't come without a record.
