Inspector Notes
A monitoring inspection was conducted on March 27, 2023 between the hours of 8:45am and 11:45am. There were 28 children present, with 7 staff supervising within the supervision guidelines. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, child and staff records, medication, special care and emergencies and nutrition. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

7Evidence: Child #4 (DOB 10/25/21) record did not have additional immunizations. Last update was 4/26/22.
Evidence: Child #1 (date of enrollment 8/1/22) did not have documentation of a physical examination by or under the direction of a physician at the time of inspection (3/27/23).
Evidence: The large cabinet in the Little Ducks classroom (ages 2-3 years) was unlocked with a bottle of bleach water solution sitting on the bottom shelf.
Evidence:
1) The licensing inspector observed all the children in the Caterpillar / Infant classroom (ages 11-15 months) were confined for 35 minutes. Seven children were in high chairs and 3 children were in bouncers from 8:45am - 9:20am.
2) Seven children were confined in high chairs as they were waiting for breakfast. Breakfast arrived at 9:20am.
3. Staff #7 stated the children were sitting in the high chairs because they were waiting on breakfast and it was late.
Evidence:
1. The inspector observed four children's hands were not washed after diapering in the Caterpillar's classroom.
2. Staff #5 did not wash Child #6 and Child #3 hands after diapering.
3. Staff #6 did not wash Child #8 hands after diapering.
4. Staff #7 did not wash Child #7 hands after diapering.
Evidence:
1. Staff #5 did not clean the diapering surface with soap and water after diapering Child #3 and Child #6. Staff #5 only used a bleach solution to clean the diapering surface.
2. Staff #6 did not clean the diapering surface with soap and water after diapering Child #8. Staff #6 only used a bleach solution to clean the diapering surface.
3. Staff #7 did not clean the diapering surface with soap and water after diapering Child #7. Staff #7 only used a bleach solution to clean the diapering surface.
Evidence: Child #2 did not have documentation of viewing proof of ID and age in their file.
