Inspector Notes

An unannounced monitoring inspection was conducted March 25, 2024 between 10:30am and 12:30pm. There were 25 children present with eight (8) staff supervising within the supervision guidelines. The children were observed playing outside, diapering and toileting procedures, handwashing, art, sleeping and playing with other age-appropriate toys and materials. Positive interactions were observed during this inspection. The inspector reviewed compliance in the areas of administration, required posted documents, physical plant, staffing and supervision, programing, medication, evacuation drill logs, menu, outside play area, attendance, child and caregiver records, and emergency procedures. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not obtain Criminal History Records check prior to the 1st day of employment.
Evidence:
1. The documentation for the Criminal History Records check is dated 08/09/23 and date of hire for Staff #1 was 07/25/23.
Plan of Correction: I will be sure to take care of the background checks on all new employees before their date of hire.
Standard 8VAC20-780-160-A
Based on record review, the center failed to obtain documentation of tuberculosis screening of each staff at the time of employment and prior to coming into contact with children.
Evidence: The documentation of a TB screening was dated 08/01/23 and the date of hire for Staff #1 was 07/25/23.
Plan of Correction: I will make sure all new employees have a completed TB screening on their date of hire.
Standard 8VAC20-780-520-C
Based on record review, the facility did not follow requirements for using diaper ointment or cream. REPEAT VIOLATION.
Evidence: The facility did not obtain written authorization for diaper cream ointment for child #4 and #5.
Plan of Correction: We will remove the ointment immediately and have parents update the authorization form today upon pickup.
