Inspector Notes
An unannounced renewal inspection was conducted on July 2, 2024. There were a total of 27 children with 10 staff members supervising. Children were observed sleeping, playing with age appropriate toys, and engaged in organized group activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. 5Evidence:
1. The record for Child #3 (date of enrollment 6/28/22) did not have documentation of a physical examination.
2. The director reviewed the file, and confirmed there was no physical examination in the file.
Evidence: The documentation of a TB screening results for Staff #1 was dated 6/29/22.
Evidence: The left side wall in the Preschool bathroom had a large area of peeling paint
Evidence: An electrical outlet in the Todd's 2 (Little Ducks) classroom did not have protective covers on the outlet.
Evidence: Child #2 (age 14 months) in the Infant classroom had a pink blanket covering while sleeping in a crib.
