Inspector Notes

An unannounced renewal inspection was conducted on July 2, 2024. There were a total of 27 children with 10 staff members supervising. Children were observed sleeping, playing with age appropriate toys, and engaged in organized group activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Standard 8VAC20-780-140-A
Based on record review and interview, each child did not have a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
1. The record for Child #3 (date of enrollment 6/28/22) did not have documentation of a physical examination.
2. The director reviewed the file, and confirmed there was no physical examination in the file.
Plan of Correction: The family is on vacation, but I sent an email informing the parents I need the physical no later than 7/16/24.
Standard 8VAC20-780-160-C
Based on record review, the center failed to obtain the results of a follow-up tuberculosis (TB) screening at least every two years from the date of the first initial screening or testing.
Evidence: The documentation of a TB screening results for Staff #1 was dated 6/29/22.
Plan of Correction: We will make the appointment today and will have the results no later than 7/9/24. I will review all staff records to ensure they have a new screening before their current one expires.
Standard 8VAC20-780-270-A
Based on observation, areas of the center were not maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to chipped or peeling paint; or visible cracks.
Evidence: The left side wall in the Preschool bathroom had a large area of peeling paint
Plan of Correction: We are schedule to have the walls painted on Friday, 7/5/24. We did have the area covered, but children pulled it off.
Standard 8VAC20-780-290-A-3
Based on observation, electrical outlets did not have protective covers that are of a size that cannot be swallowed by children.
Evidence: An electrical outlet in the Todd's 2 (Little Ducks) classroom did not have protective covers on the outlet.
Plan of Correction: The teacher corrected the violation immediately. I will reinforce to my teachers to keep the outlets covered when not using them.
Standard 8VAC20-780-440-L
Based on observation, comforters cannot be used by children under two years of age.
Evidence: Child #2 (age 14 months) in the Infant classroom had a pink blanket covering while sleeping in a crib.
Plan of Correction: Reinforce to teachers to use the sleep blankets. The violation was corrected during the inspection.
