Inspector Notes
A monitoring inspection was conducted on 6/24/2022 from 9:10 a.m. ? 12:20 p.m. There were 18 children present ranging in ages from 15 months to 7 years old and 5 staff supervising. The inspectors reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, nutrition, and transportation. A total of 6 child records and 5 staff records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued.

The staff record for S4 documented completion of the pre-service training on 05/29/2022. The staff record for S4 documents the date of employment as 12/15/2021.
Evidence:
The emergency drill documentation dated January 2022 ? June 2022 did not document the method used for notification of the drill; any special conditions simulated; problems encountered, if any; and weather conditions for emergency evacuation drills.
Evidence:
The record for C6 failed to contain documentation of annual parent confirmation of up-to-date information in the record as required by 8VAC20-780-420.E.3 Child, C6, has a date of first attendance documented as 8/5/2019, and there was no documentation in the record of parent confirmation that the information is up-to-date since the child was first enrolled.
