Inspector Notes
An unannounced Renewal inspection was completed today. A total of 11 children (26pts) with the Provider and 1 Assistant were present today. The children were observed in two groups, one grouping designated for the younger children. They were observed sitting at a table with the Assistant, engaged in table top activities. The older children were observed on the floor playing in smaller groups and were laying patterns out with magnets and sorting colors in muffin tins. Positive interaction was observed by staff with the children. A nice transition to a gross motor activity that consisted of dancing and jumping to music was completed. Walk thru of the upper level of the home and the lower level used for programming was conducted. A total of 6 children's records, all household records and all staff records (2) were reviewed. No medication needs at this time and no known food allergies. This inspection began at 10:15am and ended at 12:10pm. Thank you for your cooperation today. The closet located in the lower level bathroom was unlocked and had cleaning products and cans of paint that were within reach of children in care.
Evidence:
On the day of the inspection a written record of attendance was not completed.
Evidence:
1. The medication policy for Child #2 was not completed.
2. Child #3's record did not contain the provider's provisions of the emergency preparedness plan.
3. The Information to Parents was not signed in Child #6's record.
4. Child #5 began care on 09/06/2022. The authorization for emergency medical care was not obtained until 09/12/2022

The provider did not obtain and review the proof of identity for Child #2. who began care on 10/03/2022
Evidence:
Child #2 began care on 10/03/2022, documentation of immunizations were not obtained until 10/17/2022. Child #4 began care on 09/07/2022, documentation that Child #4 was adequately immunized was not obtained.
