Standard 8VAC20-800-60-B
A child's record shall contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reach, first date of attendance and policies for the administration of medications.
Child #1's record was missing the first date of attendance and a second emergency contact.
Child #2's record was missing the policy for administration of medication. The child was present and has been in care for over 2 weeks.
Plan of Correction: All of this information will be obtained and kept in their records.
Standard 8VAC20-800-780-B
Annually, the provider shall:review with the parent the emergency contact information to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.

An annual review of emergency contact information for Child #4 is overdue by a year.
Plan of Correction: The parent will review the emergency contacts for this child and will sign and date the review.
Standard 8VAC20-800-80-C
The provider shall document in the child's record the method of verification of the child's age and identity.

Documentation that the provider verified Child # 1 and #2's identity was not maintained. Child #2 has been in care for over 2 months.
Plan of Correction: I will require the parents to provide a birth certificate and will document the verification of this child.
Standard 8VAC20-800-90-A
Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

The provider did not obtain documentation that Child #5 has been adequately immunized. Child #5 has been in care for almost 3 months.
Plan of Correction: The parents will need to have the child adequately immunized or complete an exemption for immunizations.
