Inspector Notes

A monitoring inspection was initiated on 9/10/2021 and concluded on 09/15/2021. The director was contacted by telephone to initiate the inspection. There were 8 children present and 2 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-185-160-C
Based on review of two staff's records, the facility failed to ensure that all staff had the repeat TB screening/testing every two years.

Evidence:

1. Staff # 1 most recent TB was from 5/22/2019. The requirement is for the TB screening/testing to be completed every two years.
Plan of Correction: The director will have the staff person to obtain a new TB screening as soon as possible. The director will also monitor all dates.
Standard 63.2(17)-1720.1-A
Based on review of two staff's records, the facility failed to ensure that all repeat CPS background checks were completed every five years.

Evidence:
1. Staff # 1 most recent CPS background check was completed on 3/16/16. The background check is required to be repeated every five years.
Plan of Correction: The director will have the staff person to submit a new CPS check as soon as possible. The director will also monitor all dates.
