Inspector Notes

An unannounced monitoring inspection was conducted on 3/12/2024 from 11:55 am to 1:16 pm. There were two children in care, both age three, in one group with two staff.
three child records, two staff records, and other documents were reviewed. Children were observed in having lunch and napping.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
Based on record review, the center failed to obtain a physical examination for a child within 30 days of beginning attendance.

EVIDENCE:
Child 2 has a documented start date of 11/28/2023. A physical form in the record has no name on it to show if it is for Child 2 or for someone else.
Plan of Correction: Ensure Children have physical exams within 30 days.
Standard 8VAC20-780-150-B
Based on record review, the center failed to ensure that immunization documentation was signed by a physician, designee, or an official of a local health department.

EVIDENCE:
The immunization record and physical examination for Child 1 were not signed by a physician, designee, or an official of a local health department.
Plan of Correction: Be sure that all immunization forms are signed properly.

Based on record review, the center failed to ensure that staff receives 16 hours of training each year.

EVIDENCE:
1. The record for staff 1 documents that seven hours of training was done in 2023.

2. The record for staff 2 documents that 13 hours of training were done within the last employment year, from 2-10-23 through 2/9/2024.
Plan of Correction: Make sure all staff have required trainings annually.
Standard 8VAC20-780-450-A
Based on observation, the center failed to ensure that cots used by children during the rest period had both a top and bottom cover or a one-piece covering which is open on three edges.

EVIDENCE:
Children 2 and 4 were observed napping and the cots had no bottom covers. Staff 3 and 4 were not aware that bottom covers were needed.
Plan of Correction: Make sure cots are covered top & bottom.

Based on record review, the center failed to obtain some required information in children?s records.

EVIDENCE:
The record for Child 2 has no street addresses for emergency contacts.
