Inspector Notes

An unannounced, on-site monitoring /was initiated on 10/29/2024 and completed on 10/29/2024.
The on-site inspection began at 10:35 am and ended at 11:53 am. The inspector reviewed compliance in the areas listed above. There were two children present and two staff. The inspector reviewed three children?s records and two staff records on-site. This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-150-B
Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department. The immunization record for Child 3 was not signed.
Plan of Correction: From now on, the center will look for signatures when the immunization records are brought in.
Standard 8VAC20-780-530-C
There shall be two staff members who have current certification in first aid (FA) present on the premises during the center?s hours of operation. There were two staff at the center with two children in care. Staff 1?s FA training expired in June 2023. Staff 2?s record had no proof that FA training has been completed.
Plan of Correction: The staff will look for their current certifications and put copies in the files.
Standard 8VAC20-780-550-G
Documentation shall be maintained of emergency evacuation, shelter-in-place, and lockdown drills that contains all required information. The records at the center either have old dates, or incomplete dates with the year not recorded.
Plan of Correction: The center will begin using complete dates, or putting the year, on the emergency drill records.
Standard 8VAC20-780-560-G
When food is brought from home the food container must be clearly dated. The food from home for Child 2 was missing the date.
Plan of Correction: From now on, the center will put dates on food from home when it is brought in with the child.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information. The record for Child 1, enrolled for five months, was missing the following required components: city and state for both emergency contacts.
Plan of Correction: The center will obtain the information from the parents.
