During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.
Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.
Child 1 and Child 2, enrolled for two days, did not have documentation of immunizations on file.
Plan of Correction: Every child enrolled will have immunization documentation on-file.
Standard 8VAC20-780-340-C
During the stated hours of operation, there always shall be on premises, when one or more children are present, one staff member who meets the qualifications of a program leader or program director and an immediately available staff member with direct means of communication between the two of them.
On 03/03/2026, for approximately 1 hour, there was one staff member on premises with a total of 2 children (ages 2 1/2 years).
Plan of Correction: During operation hours, there shall be at least two qualified staff members on-site.
Standard 8VAC20-780-500-A
SYSTEMIC VIOLATION: Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine. Staff shall wash their hands with soap and running water before and after a diaper change and when a child uses the toilet.
Staff 1 assisted a child, aged 2 1/2 years old, with a diaper change. Staff did not wash hands before or after the diaper change. Child 1 did not wash their hands after toileting.
Plan of Correction: Child and staff shall wash hands with soap and running water after toileting.
Standard 8VAC20-780-530-C
REPEAT VIOLATION / SYSTEMIC VIOLATION: There shall be at least two staff members who have current First Aid / CPR certification on premises during the center's hours of operation, on field trips, and wherever children are in care.
Staff 1 was the only staff member on premises with First Aid / CPR certification for approximately one hour.
Plan of Correction: Staff shall have first aid/cpr certification on premises.

SYSTEMIC VIOLATION: The center shall practice shelter-in-place procedures a minimum of twice a year.
The center has only completed 1 shelter-in-place drill over the last 12 months.
Plan of Correction: The center will complete shelter-in-place twice a calendar year.

SYSTEMIC VIOLATION: The center shall practice lockdown procedures at least annually.
The center did not complete an annual lockdown drill.
Plan of Correction: The center shall practice lockdown drills annually.
Standard 8VAC20-780-60-A
REPEAT VIOLATION: The center shall have an on-site record for every child that is enrolled in the program.
The center did not have an on-site file for two children aged 2 1/2 years old. Child 1 and Child 2 have both been enrolled for approximately 2 days.
Plan of Correction: Center will ensure records for enrolled children are on-site.
