Inspector Notes
An unannounced monitoring inspection was conducted on 12/12/2022 from 10:15am to 11:25pm. At the time of entrance, 11 children were in care with 3 staff members present. Children were observed finishing snack, playing with toys, working on a Holiday art project, and playing with playdough. Interactions between the children and staff were positive. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. A selection of staff and children records, the physical space, evacuation drills, emergency preparedness plan and attendance records were reviewed. 3Evidence:
1. Child #2's file (start date 9/06/2022) did not have documentation of viewing proof of the child's identity within 7 business days of the child's start date.
Evidence:
1. The center did not have a leakproof or plastic-lined storage systemin the bathroom where children have diaper/pull ups changed that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal.

1. Child #1 and Child #2 did not have permission slips from the parents to take children to the public park nearby or walks outside of the center.
