Inspector Notes
An unannounced renewal inspection was conducted on 6/03/2024 from 9:45am to 10:55am. At the time of entrance, 7 children were in care with 2 staff members present. Children were observed during a science lesson, playing in centers and painting. (2) staff records, and (2) children?s records, the physical space and evacuation drills were reviewed. The site was clean and organized. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice. Repeat violations were found and are identified within this report.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Violations
4Based on observation and interview, the center did not maintain, in a way that is accessible to
all staff who work with children, a current written list of all children's allergies,
sensitivities, and dietary restrictions documented in the allergy plan.
Evidence:
1. Record review of one child's physical documented a child with a food allergy.
2. Staff stated a current written list of all children's allergies, sensitivities, and dietary
restrictions does not exist. Staff stated the parent updated the child's information and the child is no longer allergic to food.
Based on observation and record review, the center did not have emergency evacuation and
shelter-in-place procedures or maps posted in a location conspicuous to staff and children on each floor of the building.

level, used by children.

1. Staff stated the May 2024 evacuation drill was completed, but had not documented the drill.
Evidence:
1. Child #2 (start date 9/08/2021) was missing documentation of child updates and confirmation of up to date information in the child's record at least annually and the permission slip to take the child off premises.
