At least every two years from the date of the first initial screening or testing, staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.
Staff #2's record did not contain documentation of a follow up of a TB screening within two years. The screening was overdue by eight weeks.

Each center shall maintain and keep at the center a separate record for each child enrolled which shall contain the following information: Work phone number and place of employment of each parent who has custody.
Child #1 has been enrolled for 12 weeks; work phone numbers for both parents were not documented.
Child #2, Child #3, Child #4, and Child #5 were all missing one parent work phone number. Child #2 has been enrolled for 12 weeks. Child #3 and Child #5 has been enrolled for 14 months. Child #4 has been enrolled for eight months.
