Inspector Notes

An unannounced, onsite monitoring inspection was initiated and completed on 11/20/2024. The on-site inspection began at 10:15 am and ended at 12:00 pm. The inspector reviewed compliance in the areas listed above. There were 43 children present and 11 staff. The inspector reviewed 5 children?s records and 5 staff records.

The inspection included: document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six months for children under the age of two years.

The immunization records for Child 3 (22 months), who has been attending for 12 months, were not updated once every six months.
Plan of Correction: Will reach out to the parent to get updated.
Standard 8VAC20-780-560-F-4
Children three years of age or younger may not be offered foods that are considered to be potential choking hazards.

The children in the Walkers Class (13 months-16 months) were served fruit cocktail that contained whole grapes with their lunch.
Plan of Correction: Will not serve, will be removed from the menu.
