Inspector Notes

An unannounced, onsite monitoring inspection was initiated and completed on 05/01/2025. The on-site inspection began at 9:55 am and ended at 12:25 pm. The inspector reviewed compliance in the areas listed above. There were 42 children present and 14 staff. The inspector reviewed 5 children?s records and 5 staff records.

The inspection included: document review, tour of the facility, interviews, observations, and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Documentation of a negative tuberculosis screening must be submitted at the time of employment and shall have been completed within the last 30 days of the date of employment.

The tuberculosis screening for Staff 1 was dated 10 months prior to the date of employment.
Plan of Correction: Will not accept if older than 30 days.
