Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A monitoring inspection was initiated on September 27, 2021 and concluded on September 27, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 30 children present, ranging in ages from three months to four years, with seven staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of children's records, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within one month after attendance.

The record for child #1 (date of enrollment 08/23/2021) did not have a physical examination.
Plan of Correction: Request physical examination from parent.
