Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on October 8, 2024. The on-site inspection began at 9:20 am and ended at 11:40 am. The inspector reviewed compliance in the areas listed above. There were 61children present and 10 staff. The inspector reviewed 6 children?s records and 6 staff records on-site. This inspection included a tour of the facility, interviews, document review, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on October 19, 2024. A POC submitted after this date will not appear on the public website.
Violations
5The record for staff #1 did not have documentation that orientation training was completed.
In the Early toddler classroom, an outlet cover plate was missing from an electrical outlet in the back of the classroom near the door.
A medication authorization, for a prescription medication, was observed for Child #6. The center did not have written authorization from the child?s physician and parent for a long-term medication.
An injury report dated 10/01/2024, did not include the time of the incident, the date and time that the parent was notified, and the method in which the parent was notified.
1. The record for child #1 (enrolled for two months) was missing the parents work number.
2. The record for child # 3 (enrolled for two months) was missing the parent?s work contact number.
3. The record for child #4 and child #5, did not contain documentation of child updates and confirmation of up-to-date information.
