Inspector Notes
An unannounced inspection was initiated and completed on August 20, 2025, in response to a complaint received by the licensing office on August 11, 2025, relating to allegations of programs and staffing and supervision. This inspection included interviews, and observations.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
