Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A monitoring inspection was initiated on 6/4/2021 and concluded on 6/4/2021. The director was contacted by telephone to initiate the inspection. There were 22 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 new staff record submitted by the facility to ensure documentation was complete.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Violations
1Evidence:
1. Staff A (date of hire 6/1/2021) indicated on the sworn disclosure statement dated 5/20/2021 that they had lived outside the state of Virginia within the past five years.
2. There was no documentation that the required out of state sex offender registry check or out of state criminal history record check had been completed prior to hire.
4. The director verified that an out of state sex offender registry check and an out of state criminal history check were not completed for Staff A.
