This inspection was conducted by licensing staff using an alternate remote protocol and an on-site visit.
A complaint was received on 2/21/2022 regarding ratios and infant care.
The inspection was initiated on 2/22/2022 and concluded on 5/19/2022.
The evidence gathered during the investigation confirmed non-compliance with standards or law and violations were documented on the violation notice issued to the program.

Based on observation and interview, the facility failed to maintain the required ratio of 1 staff for every four infants aged birth up to 16 months. Evidence: On 2/18/2022, there were 5 infants alone with one staff person for 25 minutes.
Plan of Correction: Staff member was running late and did arrive.

Based on observation, the facility failed to provide awake infants a change in play space at least every 30 minutes. Evidence: on 2/18/2022, three infants were in bouncer seats or in chairs at the table from 9:20 a.m. to 10:44 a.m.

Based on observation, the facility did not ensure that infants who cannot hold their own bottles shall be held when fed and that bottles should not be propped. Evidence: On 2/18/2022, 3 infants were fed in an infant bouncer seat with a bottle that was propped by a blanket.
